Credit card checklist

One checklist applies to every prepaid card applicant.

• Valid picture Identification (Driver's licence, Passport or Voter's ID)
• Taxpayer Registration Number (TRN)
• Contact details for two (2) referees
• Proof of Address (A bill in name or Voters ID).

Contract or full-time employment with probationary period completed.

• Valid picture Identification (Driver's licence, Passport or Voter's ID)
• Taxpayer Registration Number (TRN)
• Contact details for two (2) referees
• Proof of Address (A bill in name or Voters ID).
• Last 3 months' payslips
• Last 3 months' payslipsOR Most recent payslip and a job letterOR A job letter and bank statement showing salary deposits for the last six months

Independent contractor or provider of professional services.

• Valid picture Identification (Driver's licence, Passport or Voter's ID)
• Taxpayer Registration Number (TRN)
• Contact details for two (2) referees
• Proof of Address (A bill in name or Voters ID).
• Income verification letter
• Last 12 months' payslips / invoices
• Last 12 months' Bank Statements (Personal Account)

Business owner not classified as a contractor or haulage operator.

• Valid picture Identification (Driver's licence, Passport or Voter's ID)
• Taxpayer Registration Number (TRN)
• Contact details for two (2) referees
• Proof of Address (A bill in name or Voters ID).
• Current Financial Statements for the business
• Auditor's / Accountant's Confirmation of monthly / annual gross and net income
• Last 12 months' Bank Statements (Personal Account)

Provides transportation services to hire purchase or distribution companies.

• Valid picture Identification (Driver's licence, Passport or Voter's ID)
• Taxpayer Registration Number (TRN)
• Contact details for two (2) referees
• Proof of Address (A bill in name or Voters ID).
• Income verification letter
• Last 12 months' payslips / invoices
• Last 12 months' Bank Statements (Personal Account)
• Proof of ownership of vehicle used for haulage contracts

Member of the Jamaica Union of Travellers Association or Jamaica Co-operative Autobus and Lorry.

• Valid picture Identification (Driver's licence, Passport or Voter's ID)
• Taxpayer Registration Number (TRN)
• Contact details for two (2) referees
• Proof of Address (A bill in name or Voters ID).
• JUTA / JCAL Letter
• Income and Expenditure statements

Business owner not classified as a contractor or haulage operator.

• Valid picture Identification (Driver's licence, Passport or Voter's ID)
• Taxpayer Registration Number (TRN)
• Contact details for two (2) referees
• Proof of Address (A bill in name or Voters ID).
• Income verification letter
• Last 12 months' payslips / invoices
• Last 12 months' Bank Statements (Personal Account)
• Proof of ownership of vehicle used for haulage contracts
